Sports Medicine Fellowship - Application Process
Applications for the Sports Medicine Fellowship must be received no later than 10/1/2010.
Prerequisites:
- Completion of an accredited Primary Care training program (FP, Peds, Internal Medicine, PM&R, or EM)
- Eligible for a VA license
- Experience with sports medicine care
Application Packet must include:
- Universal application from the AMSSM website;
- Personal statement;
- CV; and
- Two letters of reccomendation

The Selection Committee will review all completed applications.
Applicants chosen for interview will be notified by e-mail or
telephone.
